Applications
Can I apply for a dream for my child?
Promise Dreams was created in 2001 with the sole aim of helping seriously and terminally ill children, and their families.
We accept applications to help children and young people between the ages of 0 and 18 years, inclusive, and each application
is assessed to ensure the child meets our medical criteria.
How do I apply for a dream?
To apply for a dream we require the parent or legal guardian of
the child to complete and sign an application form (which you can download
or complete
online). We ask for as much detail as possible including
any other dreams that have been given by other charities and applications
for funding from any other sources. When applying for a dream, we do ask families to consider where they will raise extra funds from as the Trustees may often offer only a donation towards the total cost of the dream
We also require two letters of support. One must be from the child's GP or consultant and the second can be from the child's social worker or Community Nurse or School Teacher or Health Visitor or similar person holding a title of authority. Each letter must be on their official letter-headed paper and must outline the following:-
- Confirmation of the illness/medical condition of the children
- Confirming what the dream is and why they support that particular dream
- Confirmation that the dream would not be hazardous to the children (e.g. able to fly - overseas travel)
We also require three quotes for the dream. In the case of house alterations, holidays or laptops this is usually easy, but we appreciate that in the case of sensory or medical equipment etc this can be quite difficult. Therefore, in cases such as this, where there is only one item that is deemed suitable for the child a description of the item/s and details of supplier (perhaps a page from a catalogue) is acceptable.
For a dream holiday application we need to know the names and ages of all passengers travelling, preferred dates for travel and any special requirements (such as roll-in showers or hoist provision) in order that we can understand the potential cost of the dream holiday.
Once we receive all of information we require, each application is put before the Trustees at their monthly meeting. The Trustees use their criteria to select applications that they are able to help, and agree funding for these in line with the charity's Funding Limits.
The families are then contacted and steps are made to fulfil the dream.
If you know a child who is up to 18 years old, has a serious or terminal illness and has a specific need or treat in mind, please fill in the form here to apply for an information pack.
Alternatively, please click here to download an application form, that you can print, complete and return to us with the requested supporting information.
